Dear Valley Energy customer,

We are aware that your delivery ticket/ invoice may be left in a different place with each delivery made to your home. In an attempt to rectify this situation, I am asking for your assistance.

We would greatly appreciate you taking the time to complete the short questionnaire below. In return for your assistance, we will provide you with a five dollar coupon towards our cylinder fueling facility.
